ZAS wenst een geïntegreerde oplossing voor de realisatie van een Zorgtraject platform dat transmuraal ingezet kan worden voor de uitwerking van “patient journey’s”, een dergelijk platform is inclusief:

Integraties met het ZAS EPD (HiX)
Integraties met andere ZAS interne bronnen voor de ontsluiting van bvb facturatiedata
Integraties met huidige en toekomstige eHealth initiatieven
Ondersteuning van 3rdparty plug-ins en relevante platformen zoals het Digital Referral Prescription (DRP) platform van de overheid ter ondersteuning van telemonitoring of gelijkwaardige initiatieven
Ondersteuning van transmurale zorg waarbij het platform ook door en voor transmurale zorgpartners ingezet wordt ter ondersteuning van de gedeelde patiënt.
Toekomstbestendigheid voor diepgaande digitale dienstverlening en ondersteuning van de transmurale zorg. De opdracht wordt gegund via een mededingingsprocedure met onderhandeling.
